Group Supplemental Health Insurance Plan Benefits

  • Physician’s office visit - Pays a per-visit benefit for visits to a doctor’s office for treatment of sickness or injury.
  • Health screening - Pays a per-test amount for specified routine examinations or other preventive testing.
  • Well child visits - Pays a per-physician visit amount when your insured dependent child visits a doctor for a physical examination and appropriate immunizations during the first year.
  • Diagnostic lab tests and imaging - Pays for X-rays and laboratory tests for diagnosis and treatment.
  • Telemedicine and online health care - Get free consultations with board-certified physicians by telephone and secure email from Consult A Doctor. Telemedicine is an alternative method of gaining access to much-needed medical care, and it’s available anytime, anywhere. Four options of consultation are available 24 hours a day, seven days a week. This service also includes an interactive online health management system, including a symptom library, your electronic medical records, risk assessments and more.
  • Prescription drugs - Covers most of the commonly prescribed generic drugs at a copay of $10 or 50 percent of the drug cost. You can also purchase brand name drugs and non-copay generics at a discounted rate. The pharmacy network has providers nationwide including all major chains and most independent pharmacies.
  • Emergency room visits - Pays a per-accident amount when an injury or illness requires emergency treatment in a hospital ER.
  • Hospital admission - Pays a lump sum if an insured person is admitted as an inpatient to a hospital for treatment of an illness or injury.
  • Hospital confinement - Pays an amount per day that an insured person is charged for a room when confined as an inpatient to a hospital for treatment of an illness or injury.
  • Intensive Care Unit - Pays an additional amount per day that the insured person is confined in and charged for an Intensive Care Unit stay.
  • Surgery - Pays an amount for surgery that’s required for treatment of an illness or injury.
  • Anesthesia - Pays a scheduled amount for the administration of anesthesia during a covered surgical procedure.
  • Critical illness - Pays a lump sum when an insured person is diagnosed with a specific critical illness. Covered critical illnesses include invasive and in situ cancer, heart attack, stroke, renal failure, coronary bypass surgery and activities of daily living (ADL) deficit.
  • Accidental death - Pays a lump sum if an injury results in death.
  • Common carrier death - Pays a lump sum if an insured person dies in an accident in a vehicle that’s licensed to carry passengers. Common carriers are airplanes, trains, buses, trolleys and boats that operate on a regularly scheduled basis between predetermined points or cities.
  • Accidental dismemberment - Pays a lump sum if an injury results in the loss of a body part.
  • Dislocation and fracture - Pays a lump sum for an injury resulting in dislocation, concussion or bone fracture.
  • Dental Care - Save 20 to 60 percent on procedures from a credentialed dental professional from the CAREington International panel of more than 65,000 providers. Participating dentists offer significantly reduced rates for dental services such as exams, cleanings, dentures, root canals and crowns. Locate a provider at www.careington.com.
  • Vision care - EyeMed Vision Care offers many features to promote health and wellness, as well as to maintain your eyesight. Visit www.eyemedvisioncare.com to find providers and resources.
  • Accident Medical - Pays a specified amount in the event of an accident.
  • Hearing care - Save 30 percent on hearing evaluations and exams and 30 to 62 percent on hearing aids from any HearPO provider. More than 200 models of hearing aids are available, including the latest programmable and digital technology. Follow-up service is offered at no charge for one year. Have access to more than 1,600 HearPO offices nationwide.
