Gurantee Issue Short Term Disability (Not underwritten by AIG Companies).

This benefit features a 0 day elimination period for accidents, 7 day elimination for sickness. Benefit period is 6 months and the benefit amounts can be $1,500 per month or $3,000 per month. Income verification may be required; benefit not to exceed 66 2/3% of current income. Must be actively at work to qualify.

The AWA Dental program can be purchased with the SecureCareTM medical program or on a stand alone basis. This dental program plan has NO waiting period for preventive or basic dental benefits and a 12 month wait for major services.  Plan Highlights and features include:   Indemnity Benefits – See the dentist of your choice.  $1500 annual maximum  Low $50 deductible – waived for preventive services.  Set reimbursements for procedures – no surprises!  Cash paid directly to you or your provider.
